Drugs and Alcohol Questionnaire
Each of the following questions refers to your drug or alcohol use in the last three months. The use of the word “drugs” indicates the use of street drugs, over the counter drugs, and prescription drugs not used properly or in accordance with the doctor’s instructions.
- Do you feel you need to make a change in your alcohol and/or drug use?
- Do you use alcohol and/or drugs to “feel better”?
- Have your friends, family, or health care provider discussed their concern regarding your alcohol and/or drug use with you?
- Have you ever been involved with the law or any illegal activity because of your use of drugs and/or alcohol?
- Do you dislike yourself or feel guilty at times because of your continued use of drugs and/or alcohol?
- Have you noticed friends avoiding you because of your use of drugs and/or alcohol?
- Have you ever tried to stop using drugs and/or alcohol and experienced intense cravings or withdrawal symptoms? (shaking, headache, nausea, etc.)
- Do you sometimes wonder if you are in control of your alcohol and/or drug use?
- Do you sometimes have problems with work, school, or other responsibilities because of your drug and/or alcohol use?
- Do you have mental health problems that increase after using alcohol and/or drugs? (Depression, anxiety, hearing voices, etc.)
- Have you had problems taking prescribed medications as directed because of your drug or alcohol use?
- If you take prescription medications, do you take the prescribed medication as directed?
- Have you ever needed alcohol and/or drugs to “get yourself going” in the morning?
- Do you find yourself frequently thinking about getting or using alcohol and/or drugs?
If you answered “Yes” to any of the questions listed above, you may have a problem with drugs or alcohol. We suggest that you print a copy of this questionnaire and make an appointment with a health care professional for an assessment.
